Psychological safety as an operating condition

Estimated 2 minute readFor chief executives2 sources

Make it safe to raise a concern and useful to do so. The response to challenge matters more than a statement that everyone has a voice.

The performance question

A team under pressure needs to be able to disclose an unrealistic deadline, missing expertise or emerging error. Safety to speak is compatible with clear standards and accountability. It does not mean every proposal must be accepted or difficult feedback avoided.

What the evidence supports

NICE NG212 section 1.2.1 recommends a compassionate and inclusive environment to support psychological safety, with active leadership, peer support and communication. HSE’s Relationships standard addresses unacceptable behaviour and reporting routes.[1][2]

A practical approach

Invite concerns before a consequential commitment and ask what might make delivery unsafe or unrealistic. Thank the person for the information, investigate fairly and explain the resulting decision. Provide a route outside the immediate manager for sensitive concerns. Train leaders to challenge the work without attacking the person.

How to review progress

Review whether concerns receive action and whether staff can use the routes without retaliation. A temporary increase in reporting may reflect greater confidence, not worsening conditions. Do not use survey scores alone to declare that the culture is safe.
